Measures are defined in more details under the "PQRS measures" page, (link at the top of the Internal Affairs page.  Please complete measures for ALL patients at ALL facilities, from JULY 1, 2021 to the end of the year.   Do Not mark anything in the gray area, except where documentation accompanies a measure that is not met.   This should be a rare occurrence from an unavoidable situation.
